CONSENT TO PERSONAL DATA PROCESSING

I, the personal data subject (website visitor / client / user of https://zdorovyak.com, https://da.zdorovyak.com/, https://ai.zdorovyak.com/, https://xml.zdorovyak.com/, and subdomains of these websites, for example https://my.zdorovyak.com): __________________________________ (full name), main identity document: _________________________ (document type, series, number, date of issue, issuing authority), registered at: ________________________________, acting freely, of my own will and in my own interest, and confirming my legal capacity, in accordance with Article 9 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", hereby give consent to Individual Entrepreneur DEMIDOV EVGENY VLADIMIROVICH (INN 771471529305, OGRNIP 325774600699146) (hereinafter the "Operator") to process my personal data under the following conditions:

1. Consent is given for processing the following personal data: surname, name, patronymic, gender, phone number, and other information about me which I provide on my own initiative when sending requests to the Operator via feedback form, or for obtaining services provided by the selected tariff plan in accordance with the offer published on the Operator's website https://zdorovyak.com/.

2. Consent is given for personal data processing both with and without automation tools.

3. Purposes of personal data processing:

• concluding a service agreement and receiving services provided by the Individual Entrepreneur according to the tariff plan selected by me and specified in the Offer (Rules for rendering services - subscription service for individual clients);
• customer identification;
• providing personalized services to the customer;
• improving service quality and usability;
• developing new services and/or tariff plans;
• conducting statistical and other research based on anonymized data;
• settlements with partner(s);
• sending the customer proposals to renew the agreement for a new term and/or proposals to purchase another tariff plan.

4. This consent includes my consent for the Operator to perform the following set of actions (operations) with personal data: collection, recording, systematization, accumulation, storage, clarification (update, modification), extraction, use, transfer (provision, access), depersonalization, blocking, deletion, destruction.

5. Consent to personal data processing is confirmed by placing a consent mark in the registration form on the pages https://da.zdorovyak.com/#/login, https://ai.zdorovyak.com/login and https://xml.zdorovyak.com/app/login, hosted on the Operator's website (and its subdomains), and pressing the register / sign-in button.

6. This consent includes consent to transfer (provision, access) of personal data specified above to Yandex.Cloud LLC, registered at: 119021, Moscow, Lev Tolstoy St., 16, premises 528, for purposes of system administration and provision of hosting services to the Operator.

7. This consent is valid during the tariff plan period and for one year after its end. This consent may be withdrawn by the personal data subject by sending written notice of withdrawal to the Operator's email: support@zdorovyak.com.

8. If consent to personal data processing is withdrawn, the Operator may continue processing personal data if grounds specified in Clauses 2-9.1 and 11 of Part 1 Article 6, Part 2 Article 10, and Part 2 Article 11 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data" exist.

I confirm that I have read the provisions of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", and the Operator's Personal Data Policy. My rights and obligations in the field of personal data protection are clear to me.
